When people search "what are the side effects of a chiropractic adjustment?", they're usually bracing for bad news. Will I be sore? Could I get a headache? Is chiropractic even safe?

Those are fair questions, and we'll answer them directly. But after 30+ years in practice, I think there's a more interesting question underneath: do all "side effects" have to be negative?

Patients often first come to us because something hurts — back pain, neck pain, headaches, stiffness. As that original problem changes, so does the conversation. They tell us they're moving more freely, sleeping better, back at the gym, they have more energy, their immunity has improved, or just "feeling better." That doesn't mean the adjustment directly caused every one of those things — but it opens a genuinely interesting conversation about the spine, the nervous system, and what the research does and doesn't currently show.

What Actually Happens During an Adjustment?

A chiropractic adjustment (called spinal manipulation in the research literature) applies a controlled force to a joint. The technique used depends on the person in front of us — their presentation, history, and clinical findings. It's never one-size-fits-all.

Before recommending any care, we want to know:

  • Why have you come to see us?

  • What's your health history?

  • What does the examination show?

  • Are there reasons to modify or avoid a particular technique?

  • Does this need further investigation?

  • Is chiropractic care even the right option for you?

We'll come back to that assessment process. First, here's what someone might actually feel after an adjustment.

Common Short-Term Side Effects

Some people notice temporary reactions after spinal manipulation, including:

  • Mild muscular soreness

  • Local tenderness

  • Temporary stiffness

  • Achiness

  • Headache

  • Tiredness or fatigue

Many people notice little or nothing at all. Your own response depends on your presenting condition, general health, the technique used, and individual factors.

Are All the Effects of an Adjustment Negative?

Not necessarily — and this is where it gets interesting.

Picture someone whose back pain has been making everyday life harder. As their symptoms ease and movement gets more comfortable, there can be a ripple effect: more walking, easier exercise, more comfortable sitting at a desk, less bracing every time they bend to tie their shoes.

That raises a real question: is an adjustment purely mechanical, or does the nervous system respond too?

Researchers have identified physiological responses following spinal manipulation — but what those responses actually mean clinically is still being worked out.

Your Spine and Nervous System: More Than a "Crack"

A common misconception is that an adjustment is just about moving bones or making a sound. The body is more complex than that.

Your joints, muscles, and surrounding tissues are full of sensory receptors constantly feeding information to your nervous system from the environment around you. Your nervous system plays a central role in movement, sensation, pain processing, and the regulation of functions throughout the body.

That's why researchers are increasingly interested in what happens neurologically and biochemically after spinal manipulation. This doesn't mean every bodily function can be "fixed" by adjusting the spine — that's too big a leap. But it does mean the adjustment isn't purely a mechanical event either.

Does an Adjustment Affect the Autonomic Nervous System?

The autonomic nervous system regulates functions that happen without conscious effort — commonly described as "fight or flight" (sympathetic) and "rest and digest" (parasympathetic). The vagus nerve is a key part of the parasympathetic side, which is why you'll see online claims that adjustments "activate the vagus nerve" and improve everything from digestion to immunity.

The actual research is more measured. A systematic review of 14 trials and 618 participants found low-quality evidence that spinal manipulation did not significantly change the autonomic measures studied, compared with control or sham treatment. There was some low-quality evidence that cervical manipulation might influence one heart-rate-variability measure linked to parasympathetic activity — but the authors flagged methodological problems and called for further research.

Translation: there's a genuine research question here, but the evidence doesn't currently support saying an adjustment "resets your vagus nerve" or flips your body into rest-and-digest mode.

In saying this, a change in posture can lead to a change in the physiological effect upon the body. A forward flexed, fight or flight posture can lead to increased stress hormones. This posture will effect the atlas and the occiput, leading to a change in vagal tone and sympathetic dominance. Rather than solely ‘adjust the atlas’, one need to address the whole ecosystem.

What About Cortisol and Stress?

A 2024 systematic review update (15 trials, 737 participants) found evidence of an immediate change in cortisol following spinal manipulation compared with control interventions.

That's a real finding — but a short-term cortisol change isn't the same as proof that chiropractic "treats stress," and it doesn't tell us whether the effect is lasting or clinically meaningful. The researchers themselves said more work is needed to establish clinical relevance.

What About Inflammation?

The same review found low-quality evidence that spinal manipulation may influence certain inflammatory biomarkers, including interleukins.

Two very different statements worth separating:

  1. Research has observed changes in some inflammatory biomarkers after spinal manipulation. ✅ Supported.

  2. Chiropractic has been proven to treat inflammatory disease. ❌ Not supported.

Many of these studies involved healthy volunteers, and researchers have called for further work on the size, duration, and clinical significance of these changes. A fair summary: researchers have observed short-term changes in some inflammatory biomarkers following spinal manipulation, but what this means clinically is still under investigation.

Does Chiropractic Boost the Immune System?

This is where extra care is needed. A systematic review in JAMA Network Open (13 studies, 795 participants) found some preliminary short-term changes in selected immune biomarkers — but no clinical evidence that spinal manipulation prevents infectious disease or improves disease-specific outcomes. The authors concluded that claims linking spinal manipulation to improved immune function are currently premature.

So while the underlying physiology is an interesting research area, "get adjusted to boost your immune system" isn't a claim the evidence supports…YET!

Can Chiropractic Help You Sleep Better?

Patients sometimes tell us they're sleeping better under care. That's meaningful to them — but it doesn't prove the adjustment directly changed their sleep physiology. Throughout this whole BLOG, there is not a this for that type of care. Chiropractric care works on allowing your body to work in harmony, unencumbered by nerve system dysfunction.

There are simple explanations. If neck or back discomfort was disrupting sleep, and that discomfort improves, then finding a comfortable position, rolling over, and staying asleep can all get easier — often alongside more daytime activity and generally feeling better. That's different from claiming an adjustment treats insomnia directly.

More Energy or Clearer Thinking?

Some people report this too, and again, it's worth not jumping straight to "the adjustment caused it." Living with ongoing discomfort is tiring — it can affect activity, sleep, concentration, and mood. If musculoskeletal symptoms improve and someone moves and functions better, it's not unreasonable that they'd describe feeling better overall. That's a plausible flow-on effect, not a direct proven mechanism.

Roger Sperry’s award winning research stated that movement of the spine helps to charge the brain. Therefore, nee can hypothesise that a locked up, arthritic, misaligned spine will not charge the brain, draining the body.

What About Blood Pressure?

The evidence here is genuinely mixed. One systematic review of lower-bias trials found no statistically significant reduction in blood pressure compared with controls, and concluded there wasn't enough low-bias evidence to support spinal manipulation as a hypertension treatment. A later meta-analysis reported possible reductions in systolic (and, in some analyses, diastolic) blood pressure.

Given that disagreement, there's enough scientific interest to keep studying cardiovascular responses — but not enough evidence to advertise chiropractic as a blood pressure treatment. If you have hypertension, it should be managed with your medical team.

What About Cholesterol, Fertility, or Other Conditions?

This is where personal stories can be powerful — and misleading. Two things happening around the same time doesn't mean one caused the other. A pregnancy occurring during a course of chiropractic care doesn't establish that care caused it. A cholesterol change during care doesn't establish the adjustment caused it either. Diet, activity, weight, sleep, medication, hormones, genetics, and medical treatment can all be involved. These stories are personally meaningful, but they aren't scientific evidence of causation — a distinction that matters when discussing healthcare publicly.

In saying that, yes we have evidence, from women who were deemed infertile by the medical profession, falling pregnant. Again though chiropractic is not a cure for fertility.

Chiropractic Is More Than What Happens on the Table

Looking after yourself means looking at the whole picture — not just the 15 minutes on the table.

Before We Adjust, We Assess

This might be the most important part of this whole article: an adjustment isn't automatically the right call for every person or every condition. That's why our process starts well before anyone gets adjusted.

  1. Consultation and health history — why you're here, past injuries, relevant medical history, medications, and anything else that might shape your care.

  2. Comprehensive examination — postural assessment, chiropractic examination, orthopaedic testing, neurological assessment, and functional/movement assessment, as appropriate.

  3. Imaging, when clinically indicated — X-rays aren't routine for every patient. When your history and exam findings support it, we may refer you for imaging or other investigations.

  4. Deciding if chiropractic is the right fit — sometimes yes, sometimes with a modified approach, sometimes with further investigation, and sometimes a referral elsewhere is the right call.

The goal of assessment isn't to guarantee a result — no healthcare practitioner honestly can. It's to gather enough information to make a sound clinical decision about whether chiropractic care is appropriate for you.

So, What Are the Side Effects of a Chiropractic Adjustment?

Some people get temporary soreness, stiffness, headache, tiredness, or local discomfort after spinal manipulation. Many people notice little or nothing.

But "side effects" might not be the most interesting question here. Researchers continue to investigate what happens mechanically, neurologically, and biochemically after an adjustment — changes in cortisol, some inflammatory markers, autonomic activity, immune biomarkers, pain, and movement. Some findings are intriguing. Others are inconclusive. Plenty of questions remain open.

That's how science works. We shouldn't claim more than the evidence shows — but we also shouldn't stop asking questions just because we don't have every answer yet.

The Bigger Question: How Are You Functioning?
